Follow Up Visit After Discharge from the Hospital

To help with your transition from the hospital to back home, it will be important that a follow up appointment is scheduled with your mental health provider within one week of your discharge. You and your mental health provider will be working together to develop a plan. This plan is designed to support you and keep you healthy. One of the ways to stay healthy is to keep all of your scheduled appointments.

Use your visits to talk about issues and concerns. It is important you go to your appointments prepared. Have your list of questions with you when you see your mental health provider. This way, all of your questions will be answered at the appointment and your mental health provider can get a sense of how things are going.

Visits can be used to:

  • Talk with your mental health provider about how you are feeling. If you are struggling, it is important to let someone know.

  • Discuss an ongoing treatment plan to keep you healthy. This should include information on who to call if you need help. 

  • If medications have been prescribed, ask questions about your medications or report any problems you’re having difficulty with.
